Vital Statistics Division

Vital Statistics (Birth and Death Records) is a function of the Billing and Collection Division. The Vital Statistics Division is located at City Hall, 1st Floor, 803 W. Green Avenue.

The Bureau of Vital Statistics in Austin, Texas governs the local registrar and sets the minimum requirements for obtaining a certified copy of a birth or death record in the state of Texas. The requirements are as follows:

1.    A written application must be completed by a qualified applicant (as defined by State Law).

2.    A copy of a proper form of identification must be attached to the application. (Example: Driver's License, Social Security Card).

3.    Payment of fee as follows:

Birth Certificate    

          $22.00 Each -- Effective December 1, 2005

Death Certificate   

            $ 20.00 for 1st Certificate + $3.00 for each additional certificate included in same request effective December 1, 2005.

All requests for Birth/Death Certificates must be completed on a City of Orange application form. To obtain an application form by fax, please telephone 409/883-1081 or you may download the application below.  This application must be downloaded using Adobe Acrobat Reader.  You may download a free copy of this by clicking on the button below:

   You may fax your completed application to 409/988-7315. Faxed applications must include proper ID and must be accompanied by the following Master Card or Visa  account information:

    a.    Name on the Account.

    b.    Account Expiration Date.

    c.    Credit Card Account Number.

    d.    A call back telephone number, including area code.

All other requests for Birth/Death Certificates may be paid by money order, local personal check, Master Card or Visa. Should you have any questions, please contact our office at 409/883-1081.

BIRTH RECORDS

The fee for a certified birth record is $22.00 per copy. The fee is not refundable. Certified birth records requested before 2:00 p.m. may be obtained the same day after 4:00 p.m. Certified birth records requested after 2:00 p.m. may be obtained the next business day after 4:00 p.m.

The Bureau of Vital Statistics has made electronic access to the Bureau's database available to the City of Orange. This remote system allows the city to print a short form abstract for anyone born in Texas that is in the database. The birth years are 1926 - 1940, 1942 - present. Some birth records cannot be obtained from the remote site. They include, but are not limited to the following:

    A.    Birth years not listed above.

   B.    Very recent births (within the last four months.

   C.    Delayed or Probate births (includes adoptions).

DEATH RECORDS:

The City of Orange can issue a certified death certificate for individuals who died in the city limits of Orange, Texas during the years of 1929 thru present. The fee for a certified death record is $20.00 for the first copy and $3.00 for each additional copy of the same certificate ordered at the same time. Certified death records require at least a one-day processing period and may be obtained the next business day after 4:00 p.m.

To obtain Birth and Death records from Louisiana, you may telephone 504/568-5152 or send your written request to 325 Loyola Avenue, New Orleans, LA 70112. The cost for a Louisiana birth certificate is $15.00. The cost for a Louisiana death certificate is $5.00.
